• Chamber of commerce number, or equivalent for foreign partners;
• Legal form;
• Year of establishment;
• Brief description of the partner’s core activities;
• Brief description of the partner’s experience in the target country;
• Contact persons’s name, job title, phone, email and country;
• Check to what extent all partners already have an integrity policy in place and procedures to follow that policy.

Before you start completing this form, please complete the following annexes. You must upload the annexes with the form.

• Annex II, the Theory of Change form (optional).
